Instructions for Viewing, Downloading, and Submitting
Forms
*Microsoft Word is required in
order to display and print the forms.
Viewing the Form
To view the form, click on its hyperlinked name.
Depending on what computer and operating system you use,
you might see the form in your Web browser's window, or
the form might appear in a separate window. In either
case, you can then use Microsoft Word to print the form
on your PC's printer.
Downloading and Saving a copy of the form on your Computer
With this Web page in your browser window, right-click
on the form's hyperlinked name.
From the context menu that appears, select "Save Target
As...". In the Save As dialog box, choose a directory
to save the file in, then click "Save."
